Why I Developed the 5-Point Facial Rejuvenation

When I opened the False Creek Surgical Centre in Vancouver in the 90s, facelift surgery was one of our most common procedures. At the time, if someone wanted to address sagging skin, surgery was really the only reliable option.
Later on, treatments like Botox and dermal fillers became more widely used. They offered a less invasive way to soften lines and restore some volume. For many patients, that was a good alternative. But it wasn’t addressing the same problem, not really.
It was clear to me that there was a gap between surgery and injectables. Surgery addressed structure and position. Injectables addressed lines and volume. There wasn’t much in between. And the third element, tissue health, was often treated separately from both.
For me, the focus is on understanding what’s driving the accelerated decline in skin quality, not just what you see on the surface. From there, treatment is built around supporting tissue health and the body’s natural ability to repair, along with subtle refinements that maintain facial balance and a natural appearance. All of this is done without surgery.
The 5 Points
This is how I tend to think through a treatment plan. Not every patient needs every step, but it helps organize what I’m seeing and what to address first.
1. Understanding Your Main Concern
Every plan starts with what brought you in. I look beyond the surface to understand what’s actually causing it. What looks like a line or hollowing may come from changes in structure, collagen and elastin, or tissue position. Treatment is based on the underlying issue, not just the visible symptom.
3. Lift and Positioning
Once support is in place, the next step is repositioning. I often use thread lifts to physically reposition tissue in areas like the midface or jawline. This addresses loose tissue at its source rather than compensating with volume.
4. Balance and Refinement
Subtle enhancements are used to restore balance, including fillers for the profile or nose, and neuromodulators to soften lines.
5. Finishing Details
The face is often treated first, but the lips, neck, and hands are just as visible. When these areas are not addressed, they can contrast with the face. This step brings everything into alignment. Lip filler treatments refine shape, hydration, and colour. The neck and hands are treated to improve skin quality and support, so they better match the face.

In a more conventional approach, treatment may focus on softening lines with Botox and adding volume with fillers for a quicker change, but often with less natural results.
